Falls on the same level ranked second at $9.92 billion, followed by injuries from being struck by an object or equipment at $6.21 billion. Falls to a lower level accounted for $5.23 billion, while slips or trips without a fall totaled $3.83 billion. Together, the five leading causes accounted for $38.26 billion in annual costs. The index ranks injury causes by workers compensation costs for incidents that caused employees to miss work for more than five days. The 2026 edition reflects 2023 data, allowing time for injury costs to develop, and draws on information from Liberty Mutual, the U.S. Bureau of Labor Statistics and the National Academy of Social Insurance. The insurer also issued reports on eight industries, highlighting shared exposures and differences in their costliest injury causes. Overexertion involving outside sources was the leading cause of serious, nonfatal workplace injury costs in five of the eight industries examined: wholesale trade, retail, manufacturing, health care and social assistance, and transportation and warehousing. Overexertion and same-level falls appeared among the five costliest causes in every industry. In retail, the two categories together accounted for 54.4% of costs. Construction stood apart as the only one of the eight industries in which falls to a lower level ranked first, accounting for $2.64 billion, or 26.1% of costs. Same-level falls led in leisure and hospitality, at $1.01 billion, or 29.9%, and professional and business services, at $1.61 billion, or 20.3%. Workplace violence was a distinguishing exposure in health care and social assistance, the only one of the eight sectors where intentional injury by another person appeared among the top five causes. It ranked third, at $560 million. Manufacturing was the only sector studied with repetitive motions involving microtasks among its top five, ranking fourth at $510 million, or 7% of costs. In transportation and warehousing, overexertion costs of $1.55 billion were nearly four times the $410 million attributed to roadway crashes, which ranked fourth. Professional and business services recorded higher total roadway-crash costs, at $810 million and 10.2% of costs, compared with 8.5% in transportation and warehousing. These figures measure aggregate costs, not injury rates or fatality risk.
