S.B. 1398, referred to the Banking and Insurance Committee, would amend the state’s Fiscal Code to create the Firefighter Cancer Screening Program within the Insurance Department, along with a dedicated Firefighter Cancer Screening Fund in the State Treasury. The program would pay for screening costs not otherwise covered by a firefighter’s health insurance. Eligible firefighters — career or volunteer personnel who responded to at least 15 fire or rescue emergencies in the prior calendar year — would receive a benefits card to cover those expenses. Under the bill, firefighters would undergo a screening within their first three years of eligibility and at least once every three years thereafter. Each examination, administered by a licensed practitioner, would include a multicancer early detection test and screening, where applicable, for 18 categories of cancer, including bladder, blood, breast, lung, prostate and skin. The bill defines a cancer screening examination as one using standard or abbreviated MRI, or ultrasound where imaging is not possible and recommended by a treating physician. The measure directs the department to build electronic controls to detect potentially fraudulent card use, including tracking transactions that occur outside Pennsylvania and analyzing whether out-of-state activity suggests a recipient is not a state resident. Data gathered could be shared with the Inspector General, Auditor General and Attorney General. Firefighters’ associated health records would remain private and could not be shared with their fire company absent written consent or a governing collective bargaining agreement. To seed the fund, the bill calls for a one-time $50 million transfer from the Budget Stabilization Reserve Fund to occur no later than Jan. 1, 2027. The act would take effect in 60 days of passage.
