The Occupational Safety and Health Administration said RBG Foods Inc., which operates as Piggly Wiggly in Bowden, Georgia, tasked the worker with cleaning the grinder on Jan. 29. A co-worker stepped on the machine’s foot-control pedal, starting the grinder and pulling in the employee’s hand, OSHA said in a news release dated June 1. OSHA cited the company for a willful violation, alleging it bypassed the machine’s safety guards and exposed workers to moving parts and flying debris. The agency also issued a serious violation for failing to establish a hazardous-energy control program, commonly known as lockout/tagout, and an other-than-serious violation for failing to report the amputation to OSHA within 24 hours. Foods has 15 business days from receipt of the citations and penalties to comply, request an informal conference with OSHA’s area director or contest the findings before the Occupational Safety and Health Review Commission.
