The study, covering claims from 32 states for injuries occurring between 2015 and 2022 with two years of post-injury experience, shows that the rate of joint replacement procedures per 1,000 lost-time claims rose from 7.0 to 8.3 over the study period. Most of this growth reflects a shift from inpatient to outpatient settings, with inpatient joint replacements decreasing and outpatient procedures increasing sharply. Knee replacements made up the largest share (43%) of procedures in 2021, followed by shoulders (36%) and hips (21%). Patterns in timing also varied: hip replacements were often performed soon after injury, typically within 30 days, due to fracture care needs, while knee replacements frequently occurred more than a year post-injury, and shoulder replacements were largely completed within a year. Claims involving joint replacements were associated with “high” average total costs — between approximately $116,000 and $129,000 per claim. These cases also saw extended work absences, with temporary disability durations ranging from about 43 to 55 weeks. Early revision surgery rates (procedures to correct or replace the initial joint implant) ranged from about 1.4% for hips to nearly 4.9% for shoulders in 2021. Joint replacement surgeries were most common among male workers older than age 55, highlighting the intersection of aging workforces and musculoskeletal injuries.
