The changes, led by the department’s Office of Workers Compensation Programs, will now apply not only to injured federal workers but also to beneficiaries under the Black Lung Benefits Act, the Longshore and Harbor Workers Compensation Act and the Energy Employees Occupational Illness Compensation Program Act. Labor officials said the expansion is designed to protect worker benefits while allowing the federal government to capture additional savings through rebates from pharmaceutical manufacturers, tighter clinical management and enhanced pharmacy services. According to the department, earlier reforms launched in 2018 reduced drug spending within the program. Annual expenditures fell more than 80% from about $226.2 million in 2018 to $39.8 million in 2025, a decrease that officials attributed to stricter oversight of pharmacy benefit administrators, expanded fraud detection efforts and data-driven cost controls.
