The agency said the employers failed to provide adequate ground support at the Canyon Tunnel construction project near Knights Ferry, California, where an April 14 collapse about 200 feet inside the tunnel trapped two workers. Twyla Capurro, a 35-year-old geologist, was killed in the incident. Antioch-based Drill Tech Drilling & Shoring, which was constructing the irrigation water diversion tunnel, faces $95,000 in proposed penalties for one serious accident-related ground control violation and one “willful-serious” accident-related tunnel support violation. Clovis-based Provost & Pritchard Engineering Group, which designed the tunnel, faces $117,500 in proposed penalties for similar violations and an additional “serious” training violation. Investigators found both employers allowed work to proceed without removing or adequately supporting identified dangerous ground, according to Cal/OSHA. The more than 12,000-foot-long tunnel was being built for the South San Joaquin Irrigation District. Cal/OSHA issued the citations Sept. 30. Provost & Pritchard has appealed, the agency said. Cal/OSHA’s Bureau of Investigations is conducting a separate investigation.
